This study evaluates the performance of AI models in comparison with pathologists’ interpretations of prostate cancer Gleason grading within the Labquality EQAS virtual histopathology scheme. Using whole slide images from seven prostate biopsy cases, AI results were compared against assessments from 149 participating pathologists. While AI and human grading showed substantial agreement in several cases, discrepancies highlighted some variabilities in the Gleason scoring. Overall, the findings support AI as a complementary tool that can assist pathologists. In all cases, both participants and AI models graded the clinical outcomes similarly, ensuring patients could receive comparable treatment.
